You apply to the worker screening unit in your own state or territory, not to the Commission — the check is national, so one application to one state is the whole of it, and applying in two is the common mistake the guidance names.
Renewal runs the same way, and both an application and a renewal ask for your employer's details. A check lasts up to five years.

You will apply for or renew your NDIS worker screening check with the worker screening unit in your state or territory . The NDIS worker screening check is a national check. Workers should only apply to the worker screening unit in one state or territory.
In your application or renewal, you’ll be asked to include the details of your employer. You must include these details for first time applications and renewals .
NDIS worker screening checks are valid for up to 5 years.
